## General Forms

Access to Records Request Form

Request for access to your records. Please note formal ID will need to be sighted before access to files are given.

Application for Deferral Form

This form is to be completed by students who wish to defer their studies. Deferral of studies will be granted in compassionate or compelling circumstances. Students are required to provide documentary evidence of such circumstances.
