Access to Records Request Form

Request for access to your records. Please note formal ID will need to be sighted before access to files are given.

Application for Deferral Form - International Students

This form is to be completed by international students who wish to defer their studies. Deferral of studies will be granted in compassionate or compelling circumstances as per The Animal Academy Deferral, Suspension and Cancellation Policy and Procedure. Students are required to provide documentary evidence of such circumstances.

Application for Leave of Absence Form - International Students

This form is to be completed by international students who wish to apply for a leave of absence.

Application for Withdrawal Form - International Students

This form is to be completed by international students who wish to withdraw/cancel from their course of study. Cancellation of Studies will be granted in accordance with our Deferral, Suspension and Cancellation Policy. Further information can be found in your student handbook.

Change of Personal Details Form

This form is to be completed by students who wish update/change their address, contact details or other personal information.

Credit Transfer Application Form

Request to transfer credits from another Training Package or curriculum.
